Kibo Platform Terms of Service Version 1.0 · Effective 19 July 2026 Carebridge Vital Nexus Limited (“Kibo”, “we”, “us”) Contact: privacy@kibocare.com These Terms of Service (“Terms”) explain the rules for using the Kibo website and related patient services (the “Platform”). By creating an account or selecting “I agree”, you enter into these Terms with Kibo. Please also read our Privacy Policy. Consent for United States hosting and international data transfer is collected separately and is not part of your acceptance of these Terms. 1. Who these Terms apply to These Terms apply to invited adult users of the Platform. The Platform is currently available to users in the United States, Japan, and Hong Kong. If you are under 18, you may not create an account. If you add information about another person (for example a dependent), you confirm that you are legally allowed to do so and that the information is accurate. 2. What Kibo provides Kibo is a technology platform that helps you find and communicate with healthcare providers, send enquiries and treatment information, store and share medical documents, and receive related service messages. Kibo does not provide medical advice, diagnosis, treatment, or emergency care. If you have a medical emergency, contact local emergency services immediately. Healthcare providers you connect with through the Platform are independent; Kibo does not control their clinical decisions and does not guarantee that a provider is available, suitable, or will achieve any particular outcome. 3. Your account You must provide accurate information, keep your login details secure, and tell us promptly if you think your account has been compromised. You are responsible for activity under your account. We may suspend or close accounts that breach these Terms, create security or legal risk, or harm the Platform or other users. 4. Your data We store account, personal, and medical information you provide in the United States. Sharing with a healthcare provider, or use of our service providers, may involve access or transfer outside your country. Details are in the Privacy Policy and the separate US Hosting and Cross-Border Data Transfer Consent. You control which medical documents you share with a provider; that sharing is separate from the hosting consent. 5. Acceptable use You may use the Platform only for lawful purposes. You must not attempt unauthorized access, interfere with security or availability, upload unlawful or harmful content, or misuse another person’s information. 6. Ending your account You may stop using the Platform at any time. Closing your account does not automatically delete information we must keep for legal, security, audit, or dispute reasons. Deletion and privacy requests are handled under the Privacy Policy. 7. Important disclaimers To the fullest extent allowed by law, the Platform is provided “as is” and “as available”. We do not promise uninterrupted or error-free service. Except where the law does not allow us to limit liability, Kibo is not responsible for indirect or consequential losses, and our total liability for claims related to the Platform or these Terms is limited to the amount you paid us for the Platform in the 12 months before the claim, or USD 100 if you paid nothing. 8. Governing law If you live in the United States, these Terms are governed by the laws of the State of New York (excluding conflict-of-law rules), and disputes may be subject to individual arbitration and class-action waiver as described in any dispute notice we publish with these Terms. If you live in Japan or Hong Kong, these Terms are governed by the laws of Hong Kong, and Hong Kong courts have non-exclusive jurisdiction, except where mandatory local law requires otherwise. Mandatory consumer protections in your country continue to apply. 9. Changes If we make important changes to these Terms, we will publish an updated version.
